How Do I.... Register With The Practice?

To register at the practice you need to live within the practice catchment area. If you have moved into the area from another GP's practice, you need to provide your medical card or all details of your previous GP plus your NHS number.

This helps the Primary Care Trust arrange for the transfer of your medical records from your previous practice.

We would like you to complete a short health questionnaire, and we also request that you see the nurse for a health check as part of the registration procedure.

This is carried out by our practice nurse and provides us with a record of your health needs and promotes early detection of illness. Please bring a urine sample for testing at this check.

Patients who request to join the practice list are not discriminated on the grounds of sex, sexual orientation, disability, religion or race.
