District Nurses
The district nursing team is managed by qualified district
nurses. The team provides high quality nursing care for housebound people. District
nurses recognise the unique needs of the individual and will assess all aspects
affecting a person's life. A plan of care will be negotiated with themselves
and others involved with and connected to their ongoing care. District nursing
includes:
- Caring for the patient who has acute nursing needs which require
the skills of a trained nurse
- Promotion of health and the prevention of
ill-health
- Rehabilitation for independence
Examples of these groups could be:
- Caring for the dying patient
- Promotion of continence
- Wound management
- Management of patients with leg ulcers
- Management of medication
- Caring for the patient with HIV/AIDS
- Counselling
- Carer support
- Health education
- Pre- and post-operative management
- Teaching self-care skills
- Management of rehabilitation
- Management of the patient with diabetes
You can contact the team on 020 7244 5638.
